Mr Derek Hayes of Kirkby in Ashfield sadly passed away on the 11th of June in Nottingham City Hospital aged 72 years.
Derek will be missed dearly by his loving family.
The family have asked if anyone would like to make a donation it will be to The Pulmonary Fibrosis Trust in Dereks honour.
